• Character Reference Form  (Needed when opening New Accounts)
• Lost Certificate/ Passbook /Passcard Form

Please note that in addition to the Character Reference, the CIF and the Additional CIF Forms, an ORIGINAL of one of the following is required for proof of address for new accounts:
· utility bill
· telephone bill (not cellular)
· bank, building society or credit card statement

The bill or statement should have been mailed to your NEW address
recently (less than three months) and must be in your name.
A notarized copy of the identification presented to the witness is also required.
